BA.2 UPDATE: A new COVID variant, BA.2, is spreading quickly across New Jersey a month after it was first detected in the state. The BA.2 variant appears to be on its way to becoming the dominant COVID strain in the region, having more than doubled in prevalence in only a week, according to the U.S. Centers for Disease Control.
It is important that every New Jerseyan get Covid Tested now.

Chronic Care Management

Annual Wellness Visits
WellCare provides the patient with an Annual Wellness Visit (AWV). This comprehensive exam can be done one time a year with no cost to the patient in either your own home or the physician’s office. This exam is a personalized prevention plan to help prevent illness based on your current health and risk factors.
Transitional Care Management
A visit to the emergency room, admission to the hospital or a stay in a physical rehabilitation or nursing facility can be a stressful and confusing time for seniors and their families. Transitional Care is the bridge between hospital and home.
